What went wrong? Once you have determined that, steps must be taken to fix the problem and keep it from reoccurring.

- Follow the new process through and ensure the change doesn’t cause problems in new places
Processes can’t be assumed to work. They must be examined on a regular basis to keep them functioning properly.
